2020-2021 Requirements to Participate in Saints Sports

Rank One is the electronic medical records system that is both FERPA and HIPAA compliant that JPII and our partners at Youngs Physical Therapy have adopted to securely collect forms related to athlete eligibility and document athletic injuries. Both parents and students who have mobile phones should download the “Parent/Public Rank One” app.

Once you have downloaded the Rank One app, registered, and claimed your students, the forms below need to be uploaded/completed in Rank One prior to participation in practices or games. To do so, under “Your Students” tap the right arrow to the right of your student-athlete’s name and then tap on “View” next to “Forms.” A red “X” will appear next to the forms that need to be completed. To complete the forms, tap “To Fill Out The Forms Click Here” at the bottom of the list of forms.

  • Concussion Vital Signs Baseline Test conducted by Youngs Physical Therapy (your coach will notify you when/where this will take place)
  • a sports physical form dated within the last 365 days (download sports physical form)
  • Concussion Vital Signs Assessment Permission Form
  • NCISAA Consent to Participate and Release Form
  • Student-Athlete & Parent/Legal Custodian Concussion Statement

All forms listed on the Rank One Parent/Public app must be completed prior to the first official practice in order for an athlete to be listed as “eligible.”

In addition to the above, all students who wish to participate in a sport must have completed a Parent Athletic Permission Form (download) prior to the first official practice.

Pre-Participation Physical

The North Carolina Independent School Athletic Association (NCISAA) requires that Pre-Participation Physical Examinations be completed every 13 months.

We are excited to announce that ECU Sports Medicine and Youngs Physical Therapy & Sports Performance will be hosting a physical day for John Paul II Catholic High School and The Oakwood School student-athletes. Physicals are completed by appointment only.

Date: Thursday, July 23, 2020 beginning at 4:00 p.m.
Location: John Paul II Catholic High School Gymnasium (2085 Quail Ridge Road Greenville, NC 27858)
Cost: $20

Concussion Vital Signs

Concussion Vital Signs (CVS) is an annual requirement for all student-athletes at John Paul II Catholic High School.

Youngs Physical Therapy & Sports Performance will be hosting two summer CVS testing dates. CVS Testing is completed by appointment only.

Dates: Monday, June 22 or Monday, June 29 at 6:30 p.m.
Location: Youngs Physical Therapy & Sports Performance (1301 E Arlington Blvd Greenville, NC 27858)
